Heart Health Assessment

  • For individuals aged 30 and over who have not been diagnosed with cardiovascular disease.
  • Focuses on heart disease risk factors, including cholesterol, blood pressure, diabetes risk, and lifestyle.

Type 2 Diabetes Risk Evaluation (AUSDRISK) – MBS 713

  • For people aged 40–49 years with risk factors, or 18–49 years if Aboriginal or Torres Strait Islander.
  • Uses the AUSDRISK tool to assess diabetes risk and develop a plan for early intervention.

75+ Health Assessment

  • For people aged 75 years and over.
  • Reviews physical, psychological, and social function, medications, falls risk, and support needs.

Aboriginal and Torres Strait Islander Health Assessment (715 Check)

  • Available annually at any age.
  • A comprehensive check covering physical, psychological, and social well-being, with referral pathways for follow-up care.
    Includes
  • Closing the Gap (CTG) support.

GP Chronic Condition Management Plan

Manage your chronic condition with a personalised GP Care Plan.

  • If you have an ongoing health condition, your GP can coordinate your care and help you access Medicare-subsidised allied health services.

Yes, most of our doctors are permanent GPs. We encourage every patient to have their own regular GP to ensure continuity and high-quality care.

For urgent or acute issues, you may be seen by any available GP to make sure you receive timely care. After that, you can return to your regular GP for ongoing management.
